Jei nesutinkate su šiomis sąlygomis, prašome nesilankyti mūsų svetainėje, nesinaudoti mūsų turiniu ir/ar paslaugomis.
Duomenų subjektas pateikdamas savo asmens duomenis (įskaitant duomenis, kuriuos tiesiogiai ar netiesiogiai pateikia lankydamasis internetinėje svetainėje ir naudodamasis jos paslaugomis), sutinka ir neprieštarauja, kad Duomenų valdytojas juos valdytų ir tvarkytų šioje Privatumo politikoje, Duomenų subjekto sutikime nurodytais bei teisės aktuose numatytais tikslais ir tvarka. Asmenys, kurie yra jaunesni negu 16 metų, negali teikti jokių asmens duomenų per Duomenų valdytojo Internetinę svetainę. PRIVATUMO POLITIKA
1. Bendrosios nuostatos
◦ UAB „Mažoji Užupio klinika“ į. k. 281305150, buveinės adresas – Polocko g. 13 – 16, LT-01204 Vilnius (toliau – Duomenų valdytojas) šia privatumo politika (toliau – Privatumo politika) užtikrina, kad asmens duomenys tvarkomi teisėtu, sąžiningu ir skaidriu būdu, renkami tik šiame dokumente nustatytais ir aiškiai apibrėžtais tikslais bei toliau nėra tvarkomi su tais tikslais nesuderinamu būdu. Šis dokumentas suteiks Jums informaciją, kokie Jūsų duomenys gali būti renkami, kur gali būti panaudoti ir kaip yra apsaugoti. Privatumo politikos nustatytos sąlygos taikomos kiekvieną kartą apsilankius svetainėje muk.lt, nepaisant to, kokį įrenginį (kompiuterį, mobilųjį telefoną, planšetę, televizorių naudojate.
◦ Duomenų subjektas pateikdamas savo asmens duomenis (įskaitant duomenis, kuriuos tiesiogiai ar netiesiogiai pateikia lankydamasis internetinėje svetainėje ir naudodamasis jos paslaugomis), sutinka ir neprieštarauja, kad Duomenų valdytojas juos valdytų ir tvarkytų šioje Privatumo politikoje, Duomenų subjekto sutikime nurodytais bei teisės aktuose numatytais tikslais ir tvarka ar kt.).
◦ Duomenų valdytojas asmens duomenis rinks laikantis galiojančių Europos Sąjungos bei Lietuvos Respublikos teisės aktų reikalavimų bei kontroliuojančių institucijų nurodymų. Taikomos visos protingos techninės ir administracinės priemonės tam, kad surinkti duomenys apie Duomenų subjektus būtų apsaugomi nuo praradimo, neleistino naudojimo ir pakeitimų.
2. Privatumo politikoje naudojamos sąvokos
◦ Asmens duomenys – fizinio asmens duomenys, kuriuos tvarko Duomenų valdytojas ir pagal kuriuos galima identifikuoti klientą apimant, bet neapsiribojant: vardas, pavardė, elektroninio pašto adresas, telefono numeris ir kt.
◦ Duomenų subjektu šioje Privatumo politikoje laikomas Besikreipiantis asmuo, Pacientas, Kandidatas, Skambinantieji telefonu arba bet koks kitas fizinis asmuo, kurio asmens duomenis tvarko Duomenų valdytojas.
◦ Besikreipiantis asmuo– fizinis asmuo besidomintis Duomenų valdytojo teikiamomis sveikatos priežiūros teikiamomis paslaugomis ar norintis užsiregistruoti dėl sveikatos priežiūros paslaugos teikimo.
◦ Kandidatas – asmuo, dalyvaujantis ar ketinantis dalyvauti Duomenų valdytojo vykdomoje personalo atrankoje.
◦ Pacientas – asmuo, sudaręs sutartį su Duomenų valdytoju dėl sveikatos priežiūros (odontologijos) paslaugų teikimo.
◦ Duomenų subjekto sutikimas – bet koks laisva valia duotas, konkretus ir nedviprasmiškas tinkamai informuoto duomenų subjekto valios išreiškimas pareiškimu arba vienareikšmiais veiksmais, kuriais jis sutinka, kad būtų tvarkomi su juo susiję asmens
◦ Duomenų tvarkymas – bet kuris su asmens duomenimis atliekamas veiksmas: rinkimas, užrašymas, kaupimas, saugojimas, keitimas (papildymas ar taisymas), teikimas, naudojimas, naikinimas ar kitoks veiksmas arba veiksmų rinkinys.
◦ Duomenų tvarkytojas – fizinis ar juridinis asmuo, kuris padeda Duomenų valdytojui, pagal jo suteiktus įgaliojimus, įgyvendinti nustatytus tikslus.
◦ Slapukas (angl. cookie) – nedidelė tekstinės informacijos dalelė, kuri automatiškai sukuriama naršant Svetainėje ir yra saugoma Jūsų kompiuteryje ar kitame įrenginyje.
◦ Tiesioginė rinkodara – veikla, skirta paštu, telefonu arba kitokiu tiesioginiu būdu siūlyti asmenims prekes ar paslaugas ir (arba) teirautis jų nuomonės dėl siūlomų prekių ar paslaugų.
3. Asmens duomenų rinkimas, tvarkymas, saugojimas
◦ Pateikdamas savo asmens duomenis, Duomenų subjektas sutinka ir neprieštarauja, kad Duomenų valdytojas juos valdytų ir tvarkytų šioje Politikoje bei teisės aktuose numatytais tikslais, priemonėmis ir tvarka.
◦ Jei Duomenų subjektas nesutinka su šia Politika ir joje aprašytu asmens duomenų tvarkymu, jis neturėtų lankytis Interneto svetainėje ir/ar naudotis Duomenų valdytojo paslaugomis.
◦ Pateikdamas asmens duomenis, Duomenų subjektas suteikia Duomenų valdytojui teisę rinkti, kaupti, sisteminti, naudoti ir tvarkyti šioje Politikoje numatytais tikslais visus asmens duomenis, kuriuos šis tiesiogiai ar netiesiogiai pateikė lankydamasis Interneto svetainėje ir naudodamasis jos paslaugomis.
◦ Duomenų subjektas yra atsakingas už tai, kad jo pateikiami duomenys būtų tikslūs, teisingi ir išsamūs. Sąmoningas neteisingų duomenų įvedimas yra laikomas Politikos pažeidimu. Jeigu pasikeičia pateikti duomenys, Duomenų subjektas privalo nedelsdamas juos ištaisyti, o nesant galimybės tai padaryti, apie tai informuoti Duomenų valdytoją. Duomenų valdytojas jokiu atveju nebus atsakingas už žalą, atsiradusią Duomenų subjektui ir (ar) tretiesiems asmenims dėl to, jog Duomenų subjektas nurodė neteisingus ir (ar) neišsamius asmens duomenis arba nesikreipė dėl duomenų papildymo ir (ar) pakeitimo jiems pasikeitus.
◦ Tvarkydamas ir saugodamas Duomenų subjektų asmens duomenis, Duomenų valdytojas įgyvendina organizacines ir technines priemones, kurios užtikrins asmens duomenų apsaugą nuo atsitiktinio ar neteisėto sunaikinimo, pakeitimo, atskleidimo, taip pat nuo bet kokio kito neteisėto tvarkymo.
◦ Duomenų valdytojas taiko skirtingus Asmens duomenų saugojimo terminus priklausomai nuo tikslo, kuriuo remiantis tvarkomi konkretūs asmens duomenys.
◦ Išimtys iš šioje privatumo politikoje nurodytų saugojimo terminų gali būti nustatomos tiek, kiek tokie nukrypimai nepažeidžia Duomenų subjektų teisių, atitinka teisinius reikalavimus ir yra tinkamai dokumentuoti.
◦ Duomenys, reikalingi siekiant pareikšti, vykdyti arba apginti teisinius reikalavimus saugomi tiek, kiek šie yra būtini tokiems tikslams pasiekti pagal teisminę, administracinę arba neteisminę procedūrą.
4. Asmens duomenų tvarkymas odontologijos paslaugų teikimo tikslu,Duomenų valdytojas tiek automatiniu, tiek neautomatiniu būdu tvarko šiuos su duomenų subjektu susijusius asmens duomenis:
◦ vardą, pavardę;
◦ gimimo datą;
◦ asmens kodą;
◦ duomenis apie lytį;
◦ suteiktos paslaugos aprašymą;
◦ elektroninio pašto adresą;
◦ telefono numerį;
◦ gyvenamąjį adresą;
◦ duomenis apie sergamas ligas ir vartojamus vaistus;
◦ duomenis apie diagnozę, atliktus tyrimus, paskirtą gydymą ir kitus sveikatos duomenis;
◦ nuotraukas (esant paciento rašytiniam sutikimui);
◦ rentgeno nuotraukas;
◦ duomenis apie paciento profesiją ir darbovietę (draudimo kompensavimo tikslu).
• Aukščiau nurodyti asmens duomenys gaunami iš Duomenų subjekto.
• Tiek automatiniu, tiek neautomatiniu būdu tvarkomi asmens duomenys gauti odontologijos paslaugų teikimo tikslu saugomi 15 (penkiolika) kalendorinių metų nuo sutarties su duomenų subjekto sudarymo dienos ar duomenų gavimo dienos (remiantis LR sveikatos apsaugos ministro 1999-11-29 įsakymu Nr. 515).
• Duomenų valdytojas, automatiniu būdu, užklausų administravimo tikslu, kai Duomenų subjektas kreipiasi į kliniką užpildydamas el. puslapyje esančią užklausos formą arba kreipiasi į kliniką bet kuriuo paštu su muk.lt duomenu, tvarko šiuos interesantų (duomenų subjektų) asmens duomenis:
◦ vardą, pavardę;
◦ telefono numerį;
◦ elektroninio pašto adresą;
◦ komentarą/užklausą.
• Šiuo tikslu gauti asmens duomenys yra saugomi 2 (dvejus) kalendorinius metus nuo duomenų gavimo dienos.
• Šiuo tikslu duotą Sutikimą Duomenų subjektas bet kada gali atšaukti.
• Pasibaigus asmens duomenų saugojimo terminui surinkti asmens duomenys yra saugiai sunaikinami, pagal Duomenų valdytojo pasirinktą būdą (trinami, popieriniai dokumentai naikinami deginant, naudojantis konfidencialių duomenų naikinimo paslaugas teikiančių įmonių paslaugomis, ištrinami).
5. Asmens duomenų tvarkymas siekiant priminti Pacientams ar pirmam apsilankymui klinikoje užsiregistravusiems asmenims apie numatomus vizitus
◦ Duomenų valdytojas, siekdamas su sutarties su Pacientais sudarymu ir/ar vykdymu susijusio tikslo priminti Pacientams ar pirmam apsilankymui klinikoje užsiregistravusiems asmenims apie sutartus numatomus vizitus. Šiuo tikslu tvarkomi tokie asmens duomenys:
◦ Vardas;
◦ Pavardė;
◦ pašto adresas;
◦ Telefono numeris;
◦ Numatomi vizitai ir jų datos.
◦ Duomenys, tvarkomi siekiant priminti Pacientams ar pirmam apsilankymui klinikoje užsiregistravusiems asmenims apie numatomus vizitus, Duomenų valdytojo gaunami tiesiogiai iš Pacientų ar pirmam apsilankymui klinikoje užsiregistravusių asmenų.
◦ Teisinis asmens duomenų tvarkymo pagrindas yra BDAR 6 str. 1 dalies b) punktas (tvarkymas, reikalingas sutarties sudarymui bei vykdymui).
◦ Duomenų subjektai turi teisę bet kuriuo metu atšaukti duotą sutikimą dėl tiesioginės rinkodaros pranešimų siuntimo, nedarant poveikio sutikimu grindžiamo duomenų tvarkymo iki sutikimo atšaukimo teisėtumui.
6. Asmens duomenų tvarkymas siekiant priminti, jog atėjo metas tęsti pradėtą gydymą arba apsilankyti profilaktiškai
◦ Duomenų valdytojas, siekdamas teisėto intereso suteikti Pacientams tolesnes medicinines paslaugas ir skatinti Pacientus reguliariai lankytis pas odontologą ir taip užtikrinti dantų ir burnos ertmės sveikatą, tvarko Pacientų asmens duomenis priminimo apsilankyti pakartotinai tikslu. Šiuo tikslu tvarkomi tokie Pacientų asmens duomenys:
◦ Vardas;
◦ Pavardė;
◦ Telefono numeris;
◦ El. pašto adresas;
◦ Pašto adresas;
◦ Duomenys, tvarkomi siekiant priminti Pacientams apsilankyti pakartotinai, Duomenų valdytojo gaunami tiesiogiai iš Pacientų.
◦ Teisinis asmens duomenų tvarkymo pagrindas yra BDAR 6 str. 1 dalies f) punktas.
7. Asmens duomenų tvarkymas tiesioginės rinkodaros tikslu Duomenų valdytojas tvarko šiuos asmens duomenis:
◦ vardą;
◦ elektroninio pašto adresą;
◦ telefono numerį.
• Šiuo tikslu duomenys gaunami Duomenų subjektui išreiškus sutikimą registracijos metu svetainėje muk.lt arba partnerio, administruojančio internetines registracijas manodaktaras.lt svetainėje uždedant varnelę arba pasirašant sutikimą klinikoje adresu: Polocko g. 13-16, LT-01204, Vilnius, dėl duomenų panaudojimo tiesioginės rinkodaros vykdymo tikslais.
• Šiuo tikslu gauti asmens duomenys yra saugomi 5 (penki) kalendoriniai metai nuo duomenų gavimo dienos. Šiuo tikslu duotą sutikimą Duomenų subjektas gali bet kada atšaukti, nenurodydamas jokių motyvų.
8. Asmens duomenų tvarkymas kandidatų į darbo vietas atrankos tikslais
◦ Duomenų valdytojas atrankos į darbo vietas tikslais tvarko tokius Kandidatų pateiktus Asmens duomenis:
◦ Vardas, pavardė;
◦ Gimimo data;
◦ Lytis;
◦ Gyvenamosios vietos adresas;
◦ pašto adresas;
◦ Telefono numeris;
◦ Profesinė patirtis;
◦ Išsilavinimas;
◦ Specializacija;
◦ Kiti Kandidato savanoriškai pateikti jo gyvenimo aprašyme ir/ar kituose pateiktuose dokumentuose esantys duomenys.
◦ Teisinis asmens duomenų tvarkymo pagrindas yra BDAR 6 straipsnio 1 dalies a) (tvarkymas gavus Kandidato sutikimą, išreikštą pateikiant savo duomenis) ir b) (tvarkymas siekiant imtis veiksmų Kandidato iniciatyva prieš sudarant sutartį) punktai.
◦ Duomenys gaunami tiesiogiai iš Kandidatų ir nėra perduodami tretiesiems asmenims.
◦ Šiuo tikslu gauti asmens duomenys yra saugomi iki kandidatų atrankos pabaigos. Su Kandidato sutikimu duomenys gali būti saugomi iki 1 kalendorinių metų nuo duomenų gavimo dienos. Šiuo tikslu duotą sutikimą Duomenų subjektas gali bet kada atšaukti, nenurodydamas jokių motyvų.
◦ Tuo atveju, jei Lietuvos Respublikos teisės aktai numato papildomų apribojimų dėl to, kokia informacija apie Kandidatus gali būti tvarkoma, Duomenų valdytojas užtikrina, kad būtų tvarkomi tik leidžiami tvarkyti Kandidatų asmens duomenys.
9. Slapukų pagalba renkami duomenys
◦ Duomenų valdytojas Interneto svetainėje naudoja slapukus (angl. cookies) tam, kad galėtų atskirti vienus Internetinės svetainės naudotojus nuo kitų. Naudodamas slapukus Duomenų valdytojas siekia užtikrinti komfortą asmenims, naršantiems Interneto svetainėje bei tobulinti pačią Interneto svetainę.
◦ Slapukai – tai mažos apimties tekstiniai failai, kurie saugomi Asmens naršyklėje ar įrenginyje (asmeniniame kompiuteryje, mobiliajame telefone ar planšetiniame kompiuteryje).
◦ Tuo atveju, kai Duomenų subjektas lankosi Interneto svetainėje ir sutinka su slapukų (angl.cookies) naudojimu, Duomenų valdytojas slapukų pagalba renka tokius lankytojų duomenis, kaip IP adresas, vietos nustatymo duomenys (informacija apie apytikrę lankytojo geografinę buvimo vietą), naudojamos naršyklės versija, ekrano rezoliucija, naudojamas įrenginys.
◦ Aukščiau nurodyti duomenys naudojami statistinių duomenų apie Interneto svetainėse apsilankiusių unikalių vartotojų skaičių ir siekiant pateikti skirtingai tikslinei grupei skirtą reklamą ar turinį.
◦ Naršantis asmuo gali ištrinti arba blokuoti slapukus, pasirinkęs atitinkamus nustatymus savo naršyklėje, leidžiančius atsisakyti visų ar dalies slapukų. Žinotina, kad naudojantis tokiais naršyklės nustatymais, kurie blokuoja slapukus (įskaitant būtinus slapukus), asmeniui gali iškilti problemų naudojantis visomis ar dalimi Interneto svetainės funkcijų.
◦ Slapukai, naudojami muk.lt svetainėje:
▪ _ga – šį slapuką naudoja „Google Analytics“, kad įvertintų vartotojo apsilankymo tikslus, parengtų ataskaitas apie tinklalapio aktyvumą svetainių operatoriams ir pagerintų kliento patirtį besilankant svetainėje;
▪ _gat – naudojami „Google Analytics“ statistinei informacijai apie interneto svetainės lankomumą rinkti;
▪ _gid – naudojami „Google Analytics“ statistinei informacijai apie interneto svetainės lankomumą rinkti;
▪ pll_language – sisteminis svetainės daugiakalbystės modulio slapukas, naudojamas, kad vartotojo naršyklė atsimintų kokią svetainės kalbą vartotojas yra pasirinkęs.

10. Duomenų subjekto teisės
◦ Jūs turite šias žemiau nurodytas duomenų subjektų teises, kurias mes įgyvendinsime, gavę iš Jūsų prašymą (el. paštu muk@muk.lt; siunčiant registruotu paštu, adresu Polocko g. 13–16, LT-01204 Vilnius) tinkamai patvirtinus savo tapatybę:
▪ susipažinti su savo asmens duomenimis ir kaip jie yra tvarkomi,
▪ reikalauti ištaisyti netikslius su Jumis susijusius asmens duomenis,
▪ reikalauti, kad būtų papildyti neišsamūs Jūsų asmens duomenys,
▪ reikalauti, kad Bendrovė ištrintų su Jumis susijusius asmens duomenis, jeigu:
▪ jie nebėra reikalingi, kad būtų pasiekti tikslai, kuriais jie buvo renkami arba kitaip tvarkomi;
▪ Jūs atšaukiate sutikimą, ir nėra jokio kito teisinio pagrindo tvarkyti Jūsų asmens duomenis;
▪ asmens duomenys buvo tvarkomi neteisėtai;
▪ Ir kitais Reglamente nustatytais pagrindais;
▪ reikalauti, kad Bendrovė apribotų Jūsų asmens duomenų tvarkymą;
▪ gauti su Jumis susijusius asmens duomenis, kuriuos Jūs pateikėte Bendrovei susistemintu, įprastai naudojamu ir kompiuterio skaitomu formatu (taip pat turite teisę paprašyti Bendrovės perkelti Jūsų asmens duomenis kitam duomenų valdytojui);
▪ nesutikti, kad su Jumis susiję asmens duomenys būtų tvarkomi (pvz., dėl tiesioginės rinkodaros ar kita).

11. Jūsų asmens duomenų teikimas trečiosioms šalims
◦ Duomenų valdytojo darbuotojų prieiga prie Jūsų asmens duomenų suteikiama tik esant būtinybei įgyvendinti savo pareigas ir tik darbuotojui įsipareigojus laikytis konfidencialumo reikalavimų.
◦ Duomenų valdytojas gali pateikti Jūsų asmens duomenis: teisėsaugos institucijoms, kuomet tam yra teisinis pagrindas, atsakant į oficialius valstybės institucijų bei teismo prašymus, tačiau tik įsitikinus šių prašymų teisėtumu kitiems tretiesiems asmenims su Jūsų sutikimu.
◦ Duomenų valdytojas gali pateikti Jūsų asmens duomenis duomenų tvarkytojams, kurie duomenų valdytojui teikia paslaugas ir tvarko Jūsų asmens duomenis duomenų valdytojo vardu ir duomenų valdytojo ar Jūsų naudai:

▪ Vilniaus teritorinė ligonių kasa, kai teikiamos paslaugos, kompensuojamos privalomojo sveikatos draudimo fondo lėšomis. Duomenys teikiami pagal vienkartinius prašymus ir remiantis pasirašyta sutartimi.
▪ Panevėžio teritorinė ligonių kasa, kai teikiamos paslaugos, kompensuojamos privalomojo sveikatos draudimo fondo lėšomis. Duomenys teikiami pagal vienkartinius prašymus ir remiantis pasirašyta sutartimi.
▪ Draudimo bendrovės, kai teikiamos paslaugos, kompensuojamos draudimo bendrovių lėšomis ar privataus kapitalo sveikatos draudimo lėšomis. Duomenys teikiami esant atskiriems duomenų subjektų prašymams.
▪ Gydymo įstaigos, kurios atsiunčia pacientus atlikti tyrimus ir kurioms tyrimai yra išsiunčiami. Duomenys teikiami bendradarbiavimo sutarčių pagrindu.
◦ UAB „Mažoji Užupio klinika“ klientų asmens duomenis dėl lizinguojamų paslaugų sudarant lizingo sutartis, tvarko kaip Duomenų tvarkytojas.
▪ UAB „Mažoji Užupio klinika“ veikia Duomenų valdytojo – UAB „Mokilizingas“, juridinio asmens kodas 124926897, Kareivių g. 11B, LT-09109, Vilnius. Teikia lizingo paslaugas, įgaliojimu pagrindu.
12. Svetainėje pateikiamos nuorodos
◦ Svetainėje yra pateikiamos nuorodos į trečiųjų šalių interneto svetaines, į teisės aktus, taip pat nuorodos į socialinius tinklus. Pažymėtina, kad trečiųjų šalių svetainėms, kurių nuorodos pateikiamos šioje svetainėje, yra taikomos šių svetainių privatumo politikos ir „Mažoji Užupio klinika“ neprisiima atsakomybės už šių svetainių pateiktos informacijos turinį, jų veiklą bei jų privatumo politikų nuostatas.

◦ UAB „Mažoji Užupio klinika”, company code 281305150, registered office address Polocko g. 13–16, LT-01204 Vilnius (hereinafter – the Data Controller), by this privacy policy (hereinafter – the Privacy Policy) ensures that personal data are processed lawfully, fairly and transparently, collected only for the purposes set out and clearly defined in this document, and not further processed in a manner incompatible with those purposes. This document will provide you with information about what data of yours may be collected, where it may be used, and how it is protected. The terms of the Privacy Policy apply each time you visit the website muk.lt, regardless of the device you use (computer, mobile phone, tablet, television).
◦ By providing their personal data (including data provided directly or indirectly when visiting the website and using its services), the Data Subject agrees and does not object that the Data Controller manages and processes such data for the purposes and in the manner specified in this Privacy Policy, in the Data Subject’s consent, and in applicable legal acts, etc.).

◦ Enquiring Person – a natural person interested in the healthcare services provided by the Data Controller or wishing to register for healthcare services.
◦ Candidate – a person participating or intending to participate in a staff selection process conducted by the Data Controller.
◦ Patient – a person who has entered into a contract with the Data Controller for the provision of healthcare (dental) services.
◦ Data Subject’s consent – any freely given, specific, informed and unambiguous indication of the data subject’s wishes by which they, by a statement or by a clear affirmative action, signify agreement to the processing of personal data relating to them.
◦ Data processing – any operation performed with personal data: collection, recording, accumulation, storage, alteration (supplementation or correction), disclosure, use, destruction or any other action or set of actions.
◦ Data processor – a natural or legal person who helps the Data Controller, under the powers granted by the Data Controller, to achieve the established purposes.
◦ Cookie – a small piece of text information that is automatically created when browsing the Website and stored on your computer or other device.
◦ Direct marketing – activity intended to offer goods or services to persons by mail, telephone or other direct means and/or to ask their opinion about the offered goods or services.
3. Collection, processing and storage of personal data
◦ By providing their personal data, the Data Subject agrees and does not object that the Data Controller manages and processes them for the purposes, by the means and in the manner provided for in this Policy and in legal acts.

4. Processing of personal data for the purpose of providing dental services. The Data Controller processes the following personal data relating to the data subject by both automated and non-automated means:

◦ date of birth;
◦ personal identification number;
◦ gender data;
◦ description of the service provided;
◦ email address;
◦ phone number;
◦ residential address;
◦ data on illnesses and medications used;
◦ data on diagnosis, examinations performed, prescribed treatment and other health data;
◦ photographs (with the patient’s written consent);
◦ X-ray images;
◦ data on the patient’s profession and workplace (for insurance reimbursement purposes).
• The personal data listed above are obtained from the Data Subject.
• Personal data obtained for the purpose of providing dental services, processed by both automated and non-automated means, are stored for 15 (fifteen) calendar years from the date of conclusion of the contract with the data subject or the date of receipt of the data (pursuant to Order No. 515 of the Minister of Health of the Republic of Lithuania of 29 November 1999).
• For the purpose of administering enquiries, when the Data Subject contacts the clinic by filling in the enquiry form on the website or contacts the clinic by any email containing muk.lt, the Data Controller processes the following personal data of interested persons (data subjects) by automated means:
◦ name, surname;
◦ phone number;
◦ email address;
◦ comment/enquiry.
• Personal data obtained for this purpose are stored for 2 (two) calendar years from the date of receipt of the data.
• Consent given for this purpose may be withdrawn by the Data Subject at any time.
• After the personal data retention period expires, the collected personal data are securely destroyed by a method chosen by the Data Controller (deleted, paper documents destroyed by burning, using confidential data destruction service providers, erased).
5. Processing of personal data in order to remind Patients or persons registered for a first visit to the clinic about upcoming appointments
◦ The Data Controller, for a purpose related to the conclusion and/or performance of a contract with Patients, seeks to remind Patients or persons registered for a first visit to the clinic about agreed upcoming appointments. The following personal data are processed for this purpose:
◦ Name;
◦ Surname;
◦ postal address;
◦ Phone number;
◦ Upcoming appointments and their dates.
◦ Data processed in order to remind Patients or persons registered for a first visit about upcoming appointments are obtained by the Data Controller directly from Patients or such registered persons.
◦ The legal basis for processing personal data is Article 6(1)(b) GDPR (processing necessary for the conclusion and performance of a contract).
◦ Data Subjects have the right to withdraw consent for receiving direct marketing messages at any time, without affecting the lawfulness of processing based on consent before its withdrawal.
6. Processing of personal data in order to remind that it is time to continue treatment that has been started or to attend a preventive visit
◦ The Data Controller, pursuing a legitimate interest in providing Patients with further medical services and encouraging Patients to visit the dentist regularly and thereby ensure dental and oral health, processes Patients’ personal data for the purpose of reminding them to attend a follow-up visit. The following Patients’ personal data are processed for this purpose:
◦ Name;
◦ Surname;
◦ Phone number;
◦ Email address;
◦ Postal address;
◦ Data processed in order to remind Patients to attend a follow-up visit are obtained by the Data Controller directly from Patients.
◦ The legal basis for processing personal data is Article 6(1)(f) GDPR.
7. Processing of personal data for direct marketing purposes. The Data Controller processes the following personal data:
◦ name;
◦ email address;
◦ phone number.
• For this purpose, data are obtained when the Data Subject expresses consent during registration on the website muk.lt or on the partner website manodaktaras.lt that administers online registrations by ticking a checkbox, or by signing a consent at the clinic at Polocko g. 13–16, LT-01204 Vilnius, for the use of data for direct marketing purposes.
• Personal data obtained for this purpose are stored for 5 (five) calendar years from the date of receipt of the data. Consent given for this purpose may be withdrawn by the Data Subject at any time without stating any reasons.
8. Processing of personal data for the purposes of selecting candidates for job positions
◦ For the purposes of selecting candidates for job positions, the Data Controller processes the following Personal data provided by Candidates:
◦ Name, surname;
◦ Date of birth;
◦ Gender;
◦ Residential address;
◦ postal address;
◦ Phone number;
◦ Professional experience;
◦ Education;
◦ Specialization;
◦ Other data voluntarily provided by the Candidate in their CV and/or other submitted documents.
◦ The legal basis for processing personal data is Article 6(1)(a) GDPR (processing based on the Candidate’s consent expressed by submitting their data) and (b) (processing in order to take steps at the Candidate’s request prior to entering into a contract).
◦ Data are obtained directly from Candidates and are not transferred to third parties.
◦ Personal data obtained for this purpose are stored until the end of the candidate selection process. With the Candidate’s consent, data may be stored for up to 1 calendar year from the date of receipt of the data. Consent given for this purpose may be withdrawn by the Data Subject at any time without stating any reasons.
◦ Where legal acts of the Republic of Lithuania provide additional restrictions on what information about Candidates may be processed, the Data Controller ensures that only Candidate personal data that are permitted to be processed are processed.
9. Data collected with the help of cookies
◦ The Data Controller uses cookies on the Website in order to distinguish one Website user from another. By using cookies, the Data Controller seeks to ensure comfort for persons browsing the Website and to improve the Website itself.
◦ Cookies are small text files stored in a Person’s browser or device (personal computer, mobile phone or tablet).
◦ When the Data Subject visits the Website and agrees to the use of cookies, the Data Controller collects via cookies such visitor data as IP address, location data (information about the visitor’s approximate geographical location), browser version used, screen resolution, and device used.

▪ Panevėžys Territorial Health Insurance Fund, when services reimbursed from the compulsory health insurance fund are provided. Data are provided under one-off requests and on the basis of a signed contract.
▪ Insurance companies, when services reimbursed by insurance companies or private health insurance are provided. Data are provided upon separate requests of data subjects.
▪ Healthcare institutions that send patients for examinations and to which examination results are sent. Data are provided on the basis of cooperation agreements.
◦ UAB „Mažoji Užupio klinika” processes clients’ personal data relating to leased services when concluding lease agreements as a Data Processor.
▪ UAB „Mažoji Užupio klinika” acts under the authorization of the Data Controller – UAB „Mokilizingas”, legal entity code 124926897, Kareivių g. 11B, LT-09109 Vilnius, which provides leasing services.
